On Saturday, October 29, 2011, rescue crews were dispatched to the intersection of Three Notch Road and Loveville Road in Oakville. This comes after a new traffic light being activated at this intersection just the day before. Fire/rescue members on scene determined that no patients would want to go to the hospital, causing all units to be returned except for one ambulance who obtained refusal of care forms from those involved in the incident.
